We will need a medical summary of your records. This can be obtained from your GP practice. This is because the form states that if a doctor who is not your GP signs the form, he must have seen your medical record to complete the medical.
HGV
This medical is needed for completion of the D4 form. Please follow this link to download the form that needs to be completed. You will need to have had an eye test and an optician to have signed this part of the form before seeing the doctor. Also a patient summary from your own GP. The appointment usually takes around 15 minutes.
Every new driver must undergo an HGV medical exam with accompanying paperwork (D4) that is submitted to the DVLA by the physician who performs the examination. Candidates are required to undergo a new medical exam every time their licence comes up for renewal

According to the gov.uk website, an HGV driver medical must be completed. From age 45, a D4 form will need to be completed every 5 years. This is until you reach 65 and then a D4 form is required annually.
